No Barriers Here Community of Practice meeting
Today, we hosted our first No Barriers Here Community of Practice meeting for our trained facilitators. What a meeting! We heard from ACP Nurse for Learning Disabilities, Phoebe Mooney, and End of Life Care Practice Facilitator and Advance Care Planning lead for Somerset, Amy Giles, who shared their experiences of using No Barriers Here in East London and Somerset. Both Phoebe and Amy demonstrated both the impact and outcomes they have received in their localities since attending the No Barriers Here facilitator education programme, including a decrease in avoidable end of life care hospital admissions, an increase in advance care plans and the development of healthcare professional education days. These are just a few of many examples of the phenomenal work that is happening in the UK through No Barriers Here!
